Please provide the following information / documents for preparation of the Vehicle Hire Agreement:
1. Driving Licence picture (front & back)
2. PCO badge picture
3. PCO paper licence picture
4. Confirm the following:
a) Your marital status (single / married)
b) Your mobile phone number
c) Alternate contact number, if any
d) your email address
e) Is your address same as the one mentioned on your driving licence?
f) Date when you first got TFL’s PCO licence (if different from the one mentioned on the PCO paper licence)?
g) Your National Insurance number (need it to check your DVLA electronic counterpart online)
h) Accidents in the last 3 (three) years (date of accident, was it your fault / not your fault and is the case closed or still open with the insurance)?
i) Any medical condition which may adversely effect your ability to drive the vehicle?
j) We need details of 2 (two) individuals to be put as your next-of-kin / reference (need their name, their relationship with you & their contact numbers). The will be contacted in case of emergency only.
